I created and administered my own survey for a project. How would I cite this survey in an APA-style paper?

Since a survey you conducted yourself is not published elsewhere by someone else, you do not cite it in the same way you cite other materials. Core- Write  a 3-5 paragraph letter to the editor (based on level) responding to the following question. 

 "Do you think people who do extreme sports would not be happy if they didn't continue to take risks? In other words, do you think they need to take risks? Why or why not?"
